The upper reaches of the Illinois River in Southern Oregon is known as the Illinois Valley region. The major community is Cave Junction and the area is traversed by Highway 199, connecting the northern California coast with Interstate 5.

Below Cave Junction, the river enters the Kalmiopsis Wilderness. This stretch is designated as a National Wild and Scenic River. Eventually, the Illinois flows into the Rogue River at Agness.
