Although the Belle Fourche River is mainly in South Dakota, it begins in Campbell County, Wyoming. The Belle Fourche River Compact, a contract negotiating the appropriation of the river's waters, was signed between Wyoming and South Dakota on March 4, 1943. South Dakota received ninety percent of the unappropriated flow.

It enters South Dakota near Belle Fourche and passes through Vale before meandering through the Black Hills, Badlands & Lakes Region to Meade County, where it joins the Cheyenne River on its way to Lake Oahe and the Missouri River.
